I exported the PDF bookmarks of my version and converted them to a CSV file.
This is not a complete CSV file, but a good starter.
You'll have to correct the pages column for multi page songs as I did it for "Omaha Celebration" page 16.
And I would recommend proof reading titles and pages.
Have fun and don't forget to re-post your reworked version.
