MobileSheets can only detect changes to the button press state while the stylus is touching the screen (that's the way the Google SDK is designed). Does the issue seem to occur if you never release the button and lift the stylus? If you release the button before lifting the stylus, does that prevent the problem? It would help to know the nature of the issue. Are you not using the BOOX stylus that came with the device? Is it a third party stylus?
The S-Pen is a different beast as Samsung makes a library available specifically for their S-Pen that allows MobileSheets to detect the button changes when air actions are enabled.
As far as the annotations editor being separate from the viewing mode, there are technical reasons and also usability reasons. Every user is not going to want to use just the pen tool, so that means there needs to be a UI in order to switch those things. It's incredibly quick to go in and out of the annotations editor though. You can assign a two finger tap to enter/exit. You can have the annotations editor automatically exit after a few seconds without input.
