I know there was another thread about this, but I think the file wasn't working. I decided to get this done in case it helped out anyone else. When you're configuring the file, in the first empty slot select "title" and in the third empty slot seltect "start page". Just leave the second slot blank.

Find attached the csv file that Sciurius' PDF2CSV extracted from the PDF bookmarks of the version that I have. It seems there is an offset of 1 page.
