You are the first user to report any issues with the annotations after version 3.8.40, so I'm going to need a lot more information/details about exactly when you see the error occur, what display mode you are using, what settings you've changed from the defaults, etc.

At the moment, it seems to be a Heisenbug: it happened in rehearsal yesterday, but not in performance today, and I can't make it happen at home. Let's consider this as "Needs Confirmation" and I'll watch for it to happen again. Making notes might be difficult as it only happens when I'm actually singing notes. Is there a specific config file I could send, if it does happen?

There isn't a config or log file you can send. I would just need to know all the events that led up to it occurring. I hope you never run into it again, but if you do, I'll probably have to spend some time theorizing about how the app could get into that state.
