Is there a way to add a CSV index to a PDF that is already in MS? I searched but could not find this topic discussed anywhere.

No; a CSV can only be used when the file is imported
Mike will probably respond with a fuller answer.
Itsme (?) may have some tools that can add the entries into the database.

It depends how you want to go about it. If you just long press the song on the library screen, tap Share->Export files and select a destination, you can get a copy of the PDF that is used. You can then put the .csv in the same directory as that PDF and then use Import->CSV or PDF Bookmarks to extract individual songs from that PDF. There isn't a way to apply a csv to an existing PDF used by a song in the library though.

So does that replace the original pdf or do I still have to delete the original entry from the MS DB as well as it's physical location (of the original)

If the name of the file isn't modified, and Settings->Storage->Create Subdirectory per Song is unchecked, then yes, it would replace the original PDF. If you don't need the original song entry in your library though, you can certainly delete it first before importing the .csv.
